Abstract
Adaptation to a chronic disease involves changes in the structure of individual experience. In this work we compared the time of mental reactivation of individual experience related to different senses in patients with chronic tension-type headache (43 subjects, 21-75 years, median 53 years, including 8 men) and healthy controls (46 subjects, 21-77 years, median 59 years, including 12 men). Following instructions on the computer screen, subjects imagined seeing and touching various objects, hearing sounds, smelling odors and tasting substances. They were asked to rate pleasantness of experienced feelings on a Likert scale from +3 to -3. It has been assumed that increased differentiation of experience related to pain and dealing with it in patients with chronic tension-type headache may slow down mental reactivation of behavior related to tactile sensations. The results showed that patients with chronic tension-type headache took significantly more time for mental reactivation of tactile sensations and their evaluation as neutral, slightly unpleasant, moderately unpleasant and very unpleasant, than healthy controls. No difference between patients and healthy controls was observed in time of mental reactivation and evaluation of any feelings as pleasant. These results are in line with the hypothesis that the structure of individual experience in people with chronic tension-type headache contains a greater variety of behavioral models related to unpleasant tactile (including pain) sensations. We discuss methods of rehabilitation that can be used to adaptively modify the structure of individual experience of people with chronic headaches.
